To all Our Valued Patients:

 We regret to announce that due to a continuing rise in costs for supplies, personnel and postage, we are forced to amend some of our current financial practices. 

All    Co Pays / No Office Visit Coverage / Master Medical

are to be paid on the date of service. 

We accept cash, check, Visa and MasterCard. 

We want to make sure you are aware of this and prepared to avoid having to reschedule your appointment.

It is the responsibility of the patient to know and understand their insurance.  You must be aware of your co pays and deductibles.  Insurance policies differ greatly and we re unable to keep up with all policies and their level of detail.  If you  have a new policy or are unaware of its coverage, now is the time to look it over. If you do not have your insurance handbook or policy information on hand call the phone number on the back of your insurance card or contact your employer to verify your benefits and out of pocket cost.

There will be NO exceptions on this policy,

so please be prepared to pay at the time of service.
