Specially priced school and sport examinations

     We understand how expensive it can be to obtain a physical examination for students to be able to participate in a sport program or attend school.  As a community service, we are happy to provide these exams at a specially reduced rate of $20.00. 

     These exams are intended to be a basic screening examination only and do not include any treatment of any medical problems that my be detected.  If a medical concern is found on exam, the student will be required to see the personal physician or make an appointment at our clinic for an evaluation of that concern before allowing participation in the activity.

    We do not keep copies of examinations.  You should keep a copy of the exam, if you need it for an additional organization later on.  Otherwise the exam most likely needs to be repeated and charged for the a later date.  Sport and school exams are accepted during our normal office hours and without an appointment.  You should bring the required form in with you at the time of the exam.  College and employment exams are not included in this reduced rate.
